When a doctor says someone has "heart failure," does it mean the heart has completely stopped working, or is it just weakened?

No, "heart failure" does not mean the heart has completely stopped working.

The term itself sounds terrifyingly final, which causes a lot of misunderstanding, but it actually means the heart is failing to keep up with the body's demands. It is a chronic condition where the heart is still working, but it has become weakened, stiff, or less efficient at pumping blood than it should be.

Think of it like an engine that has lost its horsepower: it can still run and move the car, but it struggles to make it up a steep hill.

🔀 The Main Types: Weakened vs. Stiff

Doctors divide heart failure into two different categories based on exactly how the heart is struggling to do its job:

1. Systolic Heart Failure (A Squeezing Problem)

In this type, the heart muscle has become physically weakened, thin, or stretched out. When the heart contracts (squeezes), it doesn't have enough force to push an adequate amount of blood out into the body. This is often measured by a low Ejection Fraction (EF)—for instance, instead of pumping out a healthy 55% of its blood with each beat, a weakened heart might only pump out 35%.

2. Diastolic Heart Failure (A Filling Problem)

In this type, the heart muscle might still be completely strong, but it has become thick, rigid, and stiff (frequently caused by years of untreated high blood pressure). Because the muscle walls cannot relax and stretch, the heart chambers cannot fill up with enough blood between beats. Even though it squeezes with normal strength, it is squeezing a much smaller volume of blood.

🌊 Why Is It Called "Congestive" Heart Failure?

When the heart can't pump blood forward efficiently, blood and fluid begin to back up in the body's circulatory plumbing line, like a traffic jam on a highway. This fluid backup is called congestion, which is why you often hear it called Congestive Heart Failure (CHF).

  • Backup into the Lungs: If fluid backs up into the blood vessels of the lungs, it causes the most common symptom of heart failure: shortness of breath, especially when lying flat in bed or exerting yourself.

  • Backup into the Body: If fluid backs up into the veins returning from the lower body, gravity pulls that fluid down, leading to noticeable swelling (edema) in the ankles, feet, and legs, or a sudden increase in weight from fluid retention.

🆚 Heart Failure vs. Cardiac Arrest

To keep the terminology clear, it helps to see how heart failure contrasts with the heart completely stopping:

TermWhat It MeansIs the Person Awake?
Heart FailureA chronic condition where the heart is working but is too weak or stiff to pump blood efficiently.Yes. It is managed over years with medications and lifestyle changes.
Cardiac ArrestAn acute emergency where the heart's electrical system errors and the heart completely stops beating out of nowhere.No. The person collapses instantly and requires immediate CPR and an AED.

💊 Can It Be Treated?

While heart failure is generally a long-term condition without a simple cure, the outlook has improved dramatically. Modern medical treatments are highly effective at supporting the heart, easing symptoms, and allowing people to live active lives for many years.

Treatments frequently include ACE inhibitors or ARNI medications to relax blood vessels and make pumping easier, beta-blockers to prevent the heart from overworking, and diuretics (water pills) to help the kidneys flush out the excess fluid backup causing shortness of breath and leg swelling.
