What are the basic steps to use an Automated External Defibrillator (AED) on someone in cardiac arrest?

 Using an Automated External Defibrillator (AED) is incredibly simple by design. These devices are built for the general public—they are kept in airports, malls, and offices, and you do not need any medical training to use one.

The device itself will guide you through the process using loud, clear voice commands. If someone has collapsed, is unresponsive, and is not breathing, your immediate priority is to have someone call emergency services and fetch the nearest AED, while you begin CPR (chest compressions).

Once the AED arrives, follow these exact steps.

1.Turn on the AED:Immediate Action.

Open the case and press the Power button (some models turn on automatically when you open the lid). Once it is on, the machine will start talking to you, telling you exactly what to do next.

2.Prepare the Person's Chest:Ensure Good Contact.

Bared skin is required for the pads to work. Quickly cut or rip away clothing from the chest. If the chest is soaking wet, wipe it dry with a towel or cloth. If the person has a very thick mat of chest hair, use the small razor included in the AED kit to quickly shave the two small spots where the pads will go.

3.Apply the Sticky Pads:Follow the Visual Diagrams.

Peel the backing off the sticky pads. Look closely at the pictures printed right on the pads showing where they go:

  • Pad 1: Place on the upper right side of the chest, just below the collarbone.

  • Pad 2: Place on the lower left side of the chest, a few inches below the armpit (wrapped slightly around the side of the ribs).

4.Plug in the Connector Cable:If Not Already Attached.

If the wires aren't already plugged into the main AED machine, plug the connector cable into the flashing socket on the device.

5.Clear the Area for Analysis:Do Not Touch the Patient.

The AED will shout, "Analyzing heart rhythm. Do not touch the patient." Stop chest compressions immediately and step back. If anyone is touching the person, the machine will mistakenly read their heartbeat instead of the patient's.

6.Deliver the Shock (If Advised):Safety Protocol.

The machine will instantly calculate if a shock is needed:

  • If a shock is required: The machine will charge up and a button will start flashing. The voice will yell, "Stand clear! Press the flashing button now." Ensure no one is physically touching the patient, yell "Clear!", and press the button.

  • If no shock is advised: The machine will simply tell you it is safe to touch the patient and resume CPR.

7.Immediately Resume CPR:Do Not Remove the Pads.

Leave the pads attached to the chest. The moment the shock is delivered (or skipped), immediately resume forceful chest compressions. The AED will keep time for you, letting you do CPR for 2 minutes before it automatically checks the heart rhythm again.

🦺 3 Critical Safety Exceptions

While AEDs are incredibly safe, keep these three scenarios in mind:

  1. Water Safety: Never deliver a shock if the patient is lying directly in a puddle of water or on a wet boat deck. Move them to a dry area first. (A damp surface like wet grass is generally safe, as long as the chest itself is wiped completely dry).

  2. Medication Patches: If you see a square nicotine, pain, or heart medication patch directly where an AED pad needs to go, peel it off using a gloved hand and wipe the skin clean before pressing the pad down. Never place an AED pad directly on top of a medical patch.

  3. Pacemakers: If you feel a hard, cigarette-pack-sized lump beneath the skin near their collarbone, or see a surgical scar, they have an implanted pacemaker. Simply place the AED sticky pad at least 1 inch away from that lump; do not place the pad directly over it.
